Health cover is where takaful stops being a fiqh preference and becomes family logistics: networks, pre-approvals, dental options, and the question of what happens when a child needs a specialist. Four of Qatar's five takaful operators write individual and family medical cover, and they differ most in the dimension that should embarrass an industry: whether you can see a price before calling. Everything below is from published product pages and financial statements, verified August 4, 2026.

The transparency scoreboard first

OperatorPlansPublished prices?Published benefits detail?
General TakafulGold, Silver, Bronze, Lite, PearlYes: QAR 849 / 684 / 574 / 420 / 925 per yearThin: prices shown, benefit caps not laid out beside them
Alkhaleej TakafulSilver, Gold, Platinum (individuals and groups under 10)No on-page pricesYes: downloadable benefits table with limits
QIIC (Balsam)General, Bronze, Silver, Gold + visitor planNoCategories described; no benefit-limit table
Doha TakafulIndividuals, families, groupsNoNone: no tiers, tables, networks, or premiums published

That table is the market in miniature. General Takaful publishing five plan prices sounds trivial and is actually radical: nobody else does it, and it makes the first budget decision self-service. The frustration is symmetrical: General shows prices without caps, Alkhaleej shows caps without prices. A diligent buyer uses both: General's ladder to set the budget, Alkhaleej's table to understand what benefit levels look like, then quotes from both plus QIIC.

What each product actually covers

QIIC's Balsam range is the deepest documented benefit set: hospitalization, cancer treatment and kidney dialysis, pre- and post-hospitalization, home nursing, GP consultations, prescribed medicines, check-ups and vaccinations, with optional routine and major dental plus vision on Silver and Gold tiers. In-network treatment is cashless via direct billing; out-of-network claims are reimbursed, though individual claims outside the network still require original documents at the head office, a dated process. Standard exclusions deserve attention from young families: pregnancy-related conditions are excluded on the standard policy. Alkhaleej's three tiers scale from local to worldwide networks, with small-group eligibility (under 10 members) that captures family businesses most insurers ignore. General Takaful's five plans span budget (Lite at QAR 420) to premium (Pearl at QAR 925). Doha Takaful confirms it writes individual, family, and group medical on its QCB Islamic licence, and publishes nothing else; for retail buyers there is no reason to start where four competitors show their cards first, though its group schemes negotiated at corporate level are a different conversation.

The pool health question your premium flows into

Medical is currently the stress line in Qatari takaful economics. Alkhaleej's medical book expanded sharply behind a late-2023 fronting arrangement with an international provider, and the same book sits inside a participants' fund that lost QAR 29.3 million in FY2025, with AM Best noting corrective actions, which in plain language means premium increases are likely through 2026. General Takaful's fund carries QAR 27.2 million of accumulated deficits being worked off. QIIC, by contrast, generated QAR 36.1 million of net surplus in its life and medical segment in FY2024 and has distributed policyholder surplus every year since 1995, currently 15%. None of this decides your choice alone, but it frames expectations: buy on cover quality and the balance sheet standing behind claims, not on surplus hopes, and expect repricing pressure at the operators in deficit.

Choosing in practice

  • Fixed budget: start at General Takaful's ladder; QAR 420 a year for Lite is the cheapest published medical takaful entry in Qatar. Demand the schedule of limits before paying, since prices without caps are half a product.
  • Depth for serious illness: QIIC's Balsam Silver or Gold, for the documented cancer, dialysis, and home nursing cover plus dental and vision options; accept that pricing requires a quote call.
  • Family businesses and small groups: Alkhaleej's under-10 group eligibility is rare flexibility; the downloadable benefits table lets you compare limit-by-limit before engaging.
  • Employer cover top-ups: compare your employer plan's actual caps against these products before assuming you need anything; many only need a dental or maternity supplement where available.
  • Visitors and family visits: the mandatory inbound requirement is its own product line: General Takaful publishes QAR 50 per month, and Doha Takaful's travel.shamel.com.qa portal issues cover online. Details in our travel takaful comparison.

Questions that separate the tiers on a quote call

  • Exact annual limits: overall, inpatient, outpatient, and per-condition caps, in writing.
  • Network list for the tier quoted, and whether your actual hospital and clinic are direct-billing.
  • Maternity: covered, excluded, or waiting-period, and on which tier.
  • Pre-existing conditions: definitions, declarations, and waiting periods.
  • Claims mechanics outside the network: reimbursement timelines and documentation.

Qatari medical takaful has one price list, one benefits table, and two black boxes. Shop with the price list in one hand and the benefits table in the other.

Medical cover questions before you quote

  • I have employer cover: what gap is worth filling? Compare your employer schedule's caps against real scenarios: dental, maternity, specialist outpatient, and treatment abroad are the common holes. A targeted supplement beats duplicating inpatient cover you already hold.
  • Are pre-existing conditions ever covered? Typically excluded or subject to waiting periods and declaration; the operators differ in definitions, which is exactly why the quote-call questions in this guide exist. Non-disclosure risks the whole policy, not just the condition.
  • How do cashless networks actually work? In-network providers bill the operator directly against pre-approval; out-of-network you pay and claim reimbursement with originals. The network list for your tier, in writing, decides your real experience more than the brochure benefits do.
  • Can I cover my parents visiting for the summer? That is the mandatory inbound visitor product: General Takaful publishes QAR 50 per month and Doha Takaful issues online; both are separate from resident medical plans, per the travel comparison.
  • Individual plan or small-group through my business? Alkhaleej's under-10 group eligibility prices family businesses as groups, often advantageously; ask for both quotes if you run a licensed activity.
  • What makes premiums jump at renewal? Claims experience, age bands, and pool economics: the deficit-carrying operators flagged in this comparison face repricing pressure by construction. A renewal quote that jumps is your cue to re-run the market, not to lapse cover.

Abstract comparisons resolve quickly when run against a real household, so take a composite family: two adults, two school-age children, employer inpatient cover for the working spouse only. The gaps to price: the non-working spouse and children have no cover at all, dental is uncovered for everyone, and maternity may matter within the policy horizon. Budget pass first: General Takaful's published ladder prices a family of four between roughly QAR 1,680 (four Lite plans) and QAR 3,700 (four Pearl) a year, the only self-service budget estimate in the market, with the schedule of limits then requested in writing before paying. Depth pass second: QIIC Balsam quotes for the same four, tiered with dental on Silver or Gold for the adults, checking the standard exclusion of pregnancy-related conditions against the family's actual plans, since maternity cover in Qatari retail medical requires explicit confirmation, not assumption. Structure pass third: if the household runs a licensed business, Alkhaleej's under-10 group route may price the same four lives differently. The family then holds three comparable positions plus the employer plan, and the decision usually writes itself: cover the uncovered members at the tier the budget survives, add dental where the quotes justify it, and diarize the renewal re-shop, because the first year's diligence is reusable and the second year's premium is not guaranteed.

Operator financials and fee structures are covered in the 2026 state of play; the underlying model at takaful vs insurance.
